Problem Knowledge Couplers embrace the knowledge-in-tools approach to clinical decision support. Most medical decisions are based on a provider's ability to recall, process and apply large volumes of complex information at the point of care. Couplers enable the provider to: Capture a complete set of structured, standardized data input for each unique patient. "Couple" patient data with the wealth of medical knowledge from the best and most recent medical literature Instantaneously retrieve "coupled" information to support clinical decision-making for the diagnosis and management of each unique patient at a specific point in time. For far too long, health care has relied on an individual provider's ability to process large amounts of data, recall vast collections of facts, and apply relevant knowledge to a specific problem. Today, computer technology has allowed the application of tools that are capable of ensuring that the relevant knowledge is applied to every patient. With knowledge in tools, and not simply in the heads of providers, all necessary information can be gathered, and the relevant knowledge displayed for discussion and action. The result is that every patient's uniqueness is considered and the full breadth and depth of medical knowledge applied. Couplers are usually one of the following three types: SCREENING - identify risk factors, health habits, behaviors, and attitudes; offer options for provider and consumer action DIAGNOSTIC - to elicit crucial information about a patient's chief complaint and aid the provider in considering relevant diagnostic and test options MANAGEMENT - to present appropriate options for disease management for a unique patient
